Polmont Park Medical Group will be moving to a new IT system as part of a national GP IT reprovisioning programme.
Our go-live date is now scheduled for 𝗧𝘂𝗲𝘀𝗱𝗮𝘆 𝟮𝟴𝘁𝗵 𝗝𝘂𝗹𝘆.
The role out of the new system in June was paused by NHS Forth Valley after issues were identified with performance of the system. These issues should be resolved by our new GO-LIVE date.
During this changeover, we expect some temporary disruption to our normal services while we have limited or no access to patient records.
𝙃𝙚𝙧𝙚'𝙨 𝙬𝙝𝙖𝙩 𝙮𝙤𝙪 𝙣𝙚𝙚𝙙 𝙩𝙤 𝙠𝙣𝙤𝙬:

Routine appointments will be available up to and including Wednesday 22nd July.
Between Thursday 23rd July and Tuesday 28th July, we will only be able to offer appointments for urgent clinical problems that cannot safely wait.
Routine appointment booking will resume from Wednesday 29th July.

We will be 𝘂𝗻𝗮𝗯𝗹𝗲 to process repeat prescription requests submitted 𝗯𝗲𝘁𝘄𝗲𝗲𝗻 𝗪𝗲𝗱𝗻𝗲𝘀𝗱𝗮𝘆 𝟮𝟮𝗻𝗱 𝗝𝘂𝗹𝘆 𝗮𝗻𝗱 𝗪𝗲𝗱𝗻𝗲𝘀𝗱𝗮𝘆 𝟮𝟵𝘁𝗵 𝗝𝘂𝗹𝘆.
Please 𝗼𝗿𝗱𝗲𝗿 any repeat medication you will need during this period as 𝗲𝗮𝗿𝗹𝘆 𝗮𝘀 𝗽𝗼𝘀𝘀𝗶𝗯𝗹𝗲.
Your local pharmacy has been informed of the IT change and will be able to help if any medication issues arise.

During the transition, our team will have 𝗹𝗶𝗺𝗶𝘁𝗲𝗱 𝗼𝗿 𝗻𝗼 𝗮𝗰𝗰𝗲𝘀𝘀 𝘁𝗼 𝗰𝗹𝗶𝗻𝗶𝗰𝗮𝗹 𝗿𝗲𝗰𝗼𝗿𝗱𝘀.
Unfortunately, we will not be able to provide test results or information from your medical record until after Wednesday 29th July.
